What is a Patient-Centered Medical Home (PCMH)?

 

A Patient-Centered Medical Home is a trusting partnership between a healthcare team led by your doctor and you, the informed patient.  Under this care model, both doctor and patient agree to certain roles in a total healthcare program.  The result is better coordination of care to get you healthy and to keep you healthy. 

 

 

 

 

Communicate Electronically with Your Doctor

 

MyChart is a free internet-based tool that allows you to receive certain confidential medical information about yourself released at the discretion of your physicians and healthcare providers.

 

Whether you’re at work, on the road traveling, or at home, MyChart provides you with a high-level summary of your medical chart, and the ability to communicate with your doctor electronically.

 

You can request a MyChart activation code by calling a 24-hour Patient Care Representative at 1-800-LAKELAND, or by asking our staff at your next visit.

Keys to a Successful Partnership

 

We Will:

  • Ask you what your goal is or what you want to do to improve your health.

  • Ask you to help us plan your care and to let us know if you think you can follow the plan.

  • Give you a written copy of the care plan.

  • Offer a variety of ways of interaction, including face-to-face visits, telephone visits, and email through MyChart.

  • Remind you when tests are due so that you can receive the best quality care.

  • Use more advance electronic tools to help us provide more efficient care and communication.

The PCMH program is a partnership between you and your doctor.  To acknowledge your commitment to this partnership, we ask that you sign a letter of understanding.  You can print this letter by clicking below.  Please present the letter at your next appointment.

  • Respect you as an individual without judgments based on age, disability, race, religion, sex or sexual orientation.

  • Respect your privacy – your medical information will not be shared with anyone unless you give us permission or it is required by law.

  • Provide you with a doctor-led care team who knows you and your family.

  • Give the care you need when you need it.

  • Give care that meets your needs and fits with your goals and values.

  • Give evidence-based care that is centered on quality and safety.

  • Coordinate care between all of your medical care offices.

  • Have a doctor on call 24 hours a day, seven days a week.

  • Manage acute illness and chronic disease and give advice to help you stay healthy.

  • Tell you about your health and illnesses in a way you can understand.

We Trust You To:

 

  • Tell us what you know about your health, illnesses, needs, and concerns.

  • Take part in planning your care and follow the agreed-upon care plan.If you are unable, tell us why so we can help.

  • Call us first with all medical problems, unless in an emergency situation.

  • Tell us what medicines you are taking and ask for a refill at your office visit when you need one.

  • Let us know when you see other doctors and what medicines they put you on or change.

  • Ask other doctors to send us a report about your care when you see them.

  • Learn about wellness and how to prevent illnesses.

  • Respect us as individuals and partners in your care.

  • Keep your appointments, or call and let us know when you cannot.

  • Pay your share of the visit fees when you are seen in the office.

  • Give us feedback so we can improve our services to you.
